Pain Management

Pain management is a focused field of medicine that concentrates on the diagnosis, treatment, and ongoing medical care of individuals suffering from various forms of pain conditions. Pain can originate from various sources, and understanding the different types that may be affecting you is crucial to finding effective treatment. Pain is generally categorized into four main types:

  • Chronic Pain: This is pain that persists for longer than three months.
  • Acute Pain: This is short-term pain that arises in response to an injury or a surgery.
  • Neuropathic Pain: This is pain that’s caused by nerve damage.
  • Nociceptive Pain: This is pain that’s caused by physical damage to the body.

We can provide you with personalized treatment options tailored to your unique needs. The variety of treatments offered can be overwhelming. Your doctor will determine the most suitable treatment for your specific type of pain management. At Pain & Spine Physicians of San Antonio, we believe every pain experience is unique. That’s why we develop customized treatment plans tailored to your condition, medical history, and lifestyle needs, never a one-size-fits-all approach.

Injections

Injections for pain relief involve administering medication or implanting devices directly into the affected area. Injections can be a viable treatment option for various conditions, including chronic back pain, arthritis, and nerve pain. Here are several types of injections we may use to help manage your pain:

Nerve Blocks

Nerve blocks involve injecting certain medications near your nerves to prevent certain pain signals from reaching your brain. A nerve block injection targets a specific bundle of nerves to halt the transmission of pain signals and trick your body into thinking it doesn’t actually hurt. These injections vary depending on the patient’s needs.

Platelet Rich Plasma (PRP)

PRP injections use the patient’s own plasma platelets to repair damaged tissue. This is often used to treat sports injuries, arthritis, and even degenerative joint disease. The platelets contain growth factors that stimulate cellular regeneration and reduce inflammation.

Botox for Headaches

Injecting botox into specific muscles in the head can be an effective pain management technique for headaches and migraines. While it certainly isn’t a cure, it can reduce the frequency of chronic migraines by blocking neurotransmitters that contribute to pain.

Discography

Discography, or a discogram, is a diagnostic test where dye is injected into spinal discs to see if they’re the source of your pain. This procedure helps identify the disc in your spine that is causing your back pain. It’s a minimally invasive procedure.

Radiofrequency Ablation

Radiofrequency ablation, or RFA, is a treatment that targets specific nerves to target the pain you’re feeling. This treatment uses heat to disable pain nerves and provide long-term relief by preventing them from sending pain signals to the brain.

Spinal Cord Stimulation

Spinal cord stimulation, or SCS, involves implanting a device near the spine that sends mild electrical pulses to disrupt pain signals headed to the brain. A pulse generator is implanted under the skin. You will have a remote to control the stimulation levels. This can be a good way to manage chronic back pain.

Kyphoplasty

Kyphoplasty is a minimally invasive procedure that uses a medical balloon and medical bone cement to restore height and stability to vertebrae that have suffered compression fractures as a result of spinal tumors or osteoporosis. This outpatient procedure offers a quick recovery time, effective pain relief, and minimal complications.

Ketamine Infusions

Ketamine infusions are used to treat chronic pain and depression. The procedure involves an intravenous administration of low doses of ketamine to essentially reset the neural pain pathways in your brain. It’s almost like physically rebooting your nervous system. While there may be some side effects, this is viewed as a viable treatment for pain.

We offer Pain Management and Procedures Such as:
  • Ankle, Hip, Knee Joint Injections: Anti-inflammatory or regenerative medicine injections to help with arthritis, injuries, or chronic joint pain.
  • Botox for Headaches: Injections of Botox relax overactive muscles and nerves that contribute to chronic migraines.
  • Bone Marrow Aspiration: A source of stem cells, often used in regenerative medicine to help with healing and pain relief.
  • Caudal Steroid Injection: An injection near the tailbone to reduce lower back and leg pain, often caused by sciatica or spinal stenosis.
  • Cervical Facet Radio-frequency Neurotomy: A procedure that uses heat to disable pain nerves in the cervical (neck) facet joints. It’s great for long-term relief from arthritis-related neck pain.
  • Cervical Epidural Injection: A steroid injection into the epidural space of the neck to reduce inflammation and pain from pinched nerves, herniated discs, or spinal stenosis.
  • Coccygeal Nerve Blocks: A numbing injection to relieve pain in the tailbone area, great for injuries or chronic discomfort.
  • Disc Denervation: Similar to a rhizotomy, but focused on the nerves in spinal discs, reducing pain from damaged or degenerated discs.
  • Discography: A diagnostic test where dye is injected into spinal discs to see if they’re the source of your pain.
  • Stem Cells: Used to promote healing and tissue regeneration, stem cell therapy or PRP can help repair damaged joints, discs, and other structures.
  • Stimulation Epidural: Likely referring to spinal cord stimulation (SCS), where a device is placed in the epidural space to send electrical signals that block pain before it reaches the brain.
  • Steroid Injection: Anti-inflammatory shots are used to calm down pain in joints, nerves, or the spine.
  • Facet Joint Injections and Medial Branch Blocks: Targets small joints in the spine that can become painful due to arthritis or injury.
  • Failed Back Surgery Syndrome (Post Laminecto Syndrome): Ongoing pain after back surgery, often treated with injections, nerve blocks, spinal cord stimulators, or other pain management techniques.
  • Hardware Blocks: An injection used to determine if pain is coming from spinal implants (hardware) after surgery. If the pain improves, it may indicate that the hardware is causing discomfort.
  • Intercostal Nerve Blocks: An injection between the ribs to relieve nerve pain, often used for conditions like shingles or rib fractures.
  • Post Laminectomy Syndrome: A fancy name for continued back or leg pain after spinal surgery. Treatments include injections, nerve blocks, and spinal cord stimulation.
  • Trochanteric Bursa Injections: A shot into the hip bursa (fluid-filled sac) to relieve pain from bursitis or hip inflammation.
  • Spinal Cord Stimulation: A device is implanted near the spine to send mild electrical pulses, blocking pain signals before they reach the brain.
  • Interscalene Block: A nerve block in the neck that numbs the shoulder and upper arm, commonly used for shoulder surgery or severe shoulder pain.
  • Intra-articular Peripheral Joint Injections: Injections directly into a joint (like the wrist, elbow, or shoulder) to reduce pain and inflammation.
  • Intra-Articular Steroid Injection: A steroid injection into a joint to decrease inflammation, commonly used for arthritis and joint pain.
  • Joint Injections: Injections into joints (like the shoulder, elbow, or wrist) to reduce pain and inflammation.
  • Ketamine Infusions: Used for chronic pain and depression, ketamine helps “reset” pain pathways in the brain. It’s like giving your nervous system a fresh start.
  • Kyphoplasty: A minimally invasive procedure that restores height and stability to fractured vertebrae using a balloon and bone cement.
  • Lumbar Epidural Steroid Injection: A steroid injection in the lower back to relieve pain from herniated discs, sciatica, or spinal stenosis.
  • Lumbar Facet Block: An injection into the small joints of the lower spine to diagnose and treat back pain caused by arthritis or injury.
  • Lumbar Medial Branch Block: A test injection to numb the nerves that supply the lumbar facet joints, helping determine if they are the source of back pain.
  • Lumbar Spondylosis Radiofrequency Ablation: Uses heat to disable pain nerves, offering long-term relief from chronic back and neck pain.
  • Lumbar Sympathetic Block: Used for nerve pain in the lower body, this injection can improve blood flow and relieve conditions like CRPS.
  • Lysis of Adhesions: A procedure that breaks up scar tissue (adhesions) in the spine that can cause pain and limit movement. Think of it as freeing up stuck tissues.
  • Medial Branch Block: A diagnostic and therapeutic injection that numbs specific pain-carrying nerves in the spine.
  • Osteocool for Tumor Ablation of Spine: Uses a cooling radiofrequency technique to destroy spinal tumors while protecting healthy tissue.
  • Occipital Nerve Block: A targeted injection at the back of the head to relieve headaches and neck pain.
  • Posterior Facet Block (Rhizotomy): A treatment that disables overactive pain nerves by cutting or burning them, providing long-term relief from chronic pain.
  • Sciatic Nerve Block: An injection that numbs the sciatic nerve, often used for severe leg pain
  • Selective Nerve Root Block: Targets a specific nerve root causing pain, helping diagnose and treat pinched nerves
