What is Failed Back Surgery Syndrome?

Back surgery is often pursued when conservative treatments fail to provide relief. For many patients, procedures such as spinal fusion or laminectomy can reduce pain and restore function. Unfortunately, not all surgeries are successful. Some patients continue to experience pain even after an operation, a condition known as Failed Back Surgery Syndrome (FBSS).

At Pain & Spine Physicians of San Antonio, we specialize in helping patients manage and overcome chronic pain, including pain caused by FBSS. If you are struggling after back surgery, call us at (210) 634-1232 to schedule an evaluation.

Understanding Failed Back Surgery Syndrome

FBSS is not a single diagnosis but rather a broad term that describes ongoing or recurrent pain following spinal surgery. Instead of experiencing relief, patients may continue to feel persistent discomfort, or new symptoms may develop in the months or years after surgery.

The pain may be localized in the back, radiate down the legs, or present as burning, tingling, or numbness. For some, FBSS significantly interferes with daily activities, work, and overall quality of life.

Causes of FBSS

There are several reasons why surgery may not resolve spinal pain:

  • Scar tissue formation: Post-surgical scar tissue can compress nerves and create new sources of pain.
  • Residual or recurring disc problems: A herniated disc may not be fully corrected, or another disc may later degenerate.
  • Incorrect diagnosis: In some cases, the original pain source was not fully identified before surgery.
  • Spinal instability: Fusion surgery may not completely stabilize the spine, leaving continued movement and pain.
  • Nerve damage: Nerves that were compressed before surgery may not heal fully, or they may be damaged during the procedure.

Symptoms of FBSS

Failed Back Surgery Syndrome can present in a variety of ways, often leaving patients with ongoing discomfort that disrupts daily life. Many experience persistent or recurring lower back pain that does not improve after surgery.

The pain may radiate into the buttocks, hips, or legs, producing sciatica-like symptoms that feel sharp or shooting. Some patients also report numbness, weakness, or tingling in the extremities, which may signal continued nerve involvement.

Simple activities such as standing, walking, or sitting for extended periods can become difficult, and mobility may gradually become more limited. Together, these symptoms can significantly affect daily function and quality of life for those living with FBSS.

Treatment Options for FBSS

While it can feel discouraging to deal with pain after surgery, there are advanced treatments available. At Pain & Spine Physicians of San Antonio, we create individualized care plans that may include:

  • Medication management to reduce inflammation and nerve irritation.
  • Interventional pain procedures such as epidural steroid injections, nerve blocks, or radiofrequency ablation.
  • Spinal cord stimulation (SCS) is a minimally invasive therapy that uses electrical signals to block pain messages before they reach the brain.
  • Physical therapy and rehabilitation to strengthen the spine and improve function.
  • Discseel®, an innovative, non-surgical treatment designed to repair and seal damaged spinal discs, which can reduce pain and restore function.

In most cases, treatment focuses on controlling symptoms and restoring quality of life, rather than requiring repeat surgery.

Moving Forward After FBSS

Failed Back Surgery Syndrome can be physically and emotionally exhausting, but it does not mean you are out of options. With the right combination of interventional pain management and supportive care, many patients find meaningful relief and regain the ability to live active, fulfilling lives.

If you are living with ongoing pain after spinal surgery, don’t wait to explore your options. Call (210) 634-1232 or contact us online to schedule a consultation with Pain & Spine Physicians of San Antonio.
